All Imperial students entering Foundation Training are required to complete and submit a Transfer of Information (ToI) form [Word] as part of the process of entering Foundation School.

This process is mandatory for all Foundation Doctors regardless of the Foundation School to which they have been allocated. It is designed to allow you to highlight to your Foundation School Director (FSD) and Foundation Training Programme Director (FTPD) issues that might be of relevance to your performance as a doctor, and is also one of the processes used to safeguard patient safety.
The TOI process is a means of supporting medical students during the transition from Medical School to Foundation School and throughout the two year programme. This transition can be a steep learning curve and it is important that those who will be supervising you are aware of any factors that may affect your performance, so they can provide you with all the support you need.
Under the Medical Act 1983 universities have formal responsibility for confirming that doctors at the end of F1 are eligible for full registration. It is therefore essential that there is a two-way transfer of information between the Medical School and the Foundation School until the point of full registration with the GMC.
The TOI process aims to highlight doctors who may require additional support during the Foundation Programme. In particular, it is essential that information regarding any potential patient safety issues, significant educational, health, welfare, absences, other problems and areas of the student’s relative weakness are passed to those responsible for Foundation training.
Information on the TOI form will be endorsed by the Medical School Dean and further information may be added to the form where appropriate. There are two key components to the form:
  • Section 1: Information related to health and welfare
  • Section 2: Information related to educational progress
Health and welfare - Health and welfare information aims to highlight health and welfare issues that may be of relevance to the setting or delivery of the Foundation Programme. This allows you to manage your health in a professional manner.

Educational progression - Educational progress information outlines any additional support that may be required during the Foundation Programme. This information will be used by the Educational Supervisor to assist you in producing a personal development plan as a new Foundation doctor and to identify any additional educational support that may be needed. This also allows the medical school to meet the GMC requirement of ensuring that graduates’ areas of relative weakness are fed onto their Foundation School.

Please note:

  1. If you do not provide information relevant to your transition and training as an F1 doctor, the Medical School Dean may provide these additional details on the TOI form. Transferring relevant information may help reduce the risk of encountering difficulties throughout the F1 year.
  2. A copy of your TOI form will be retained by the FEO up to the point of your completion of the Foundation Programme.
  3. Information contained on this form will be passed from your Medical School to your allocated Foundation School before you start F1. Your Foundation School Director (FSD) will review the information provided and appropriately share section 1 (health & welfare) with your Trust Foundation Training Programme Director (FTPD) and section 2 (educational progress) with your Educational Supervisor. No information will be shared with anyone else unless you have given explicit permission. If any information on your TOI form requires action or a discussion, the FTPD may ask to meet you to discuss and agree an action plan. This will usually take place within two weeks of you commencing the programme.
  4. Your TOI form will be retained by your Foundation School for the duration of your Foundation Programme. The copy of the TOI form held by the FTPD will be returned to you or destroyed within one month of you starting the FP. The copy of the TOI held by your Educational Supervisor will be returned to you or destroyed within one month of the initial meeting with the Educational Supervisor taking place.
  5. The TOI will not be used at any point to determine your future professional progress, either from Medical School into F1, from F1 to F2 or at any time subsequent to this.
  6. The TOI is designed to allow you to highlight issues you believe may be pertinent. It is for you to determine how much specific information you give in regard to each of these. It is important to understand that withholding information that may impact on safe patient care or giving false information regarding your academic career would be inconsistent with the GMC guidance on probity, and any identification of such activity would be reported to the GMC.
