| How to order a replacement name badge (clinical course) |
| You are required to wear your name badge at all times while on clinical attachments. |
| If you have lost your badge, you will need to order a replacement from the FEO at Charing Cross campus prior to the start of the attachment. Badges cost £10 and you will need to provide us with a cheque made payable to Imperial College London. Badges take approximately 2 - 3 weeks to be delivered so please order your badge in good time for the start of the attachment. Once we have received delivery we will contact you by email. |
| Please note that you are not permitted to use any other names eg. middle or nick name other than the first and last name that you are registered with. Owing to space restrictions, the badge will consist of your title, first name and surname. |
